Pharmacology

  • Active form: Calcitriol
  • Binds to Vitamin D receptors (VDRs)
  • Fat-soluble with a half-life of about 50 days
  • Absorbed in the small intestine
  • Metabolized to 25-hydroxy Vitamin D3 (Calcidiol) in the liver
  • Second hydroxylation to 1,25-dihydroxy Vitamin D3 (Calcitriol) occurs in the kidney
  • Increases calcium absorption

Dosage and Administration

  • Adult dosage: 40000 IU once weekly for 7 weeks for deficiency treatment
  • Adult maintenance therapy: 1400-2000 IU/day
  • Children (12-18 years) dosage for deficiency treatment: 20000 IU once every 2 weeks for 6 weeks
  • Infant and child syrup dosage for Cholecalciferol deficiency
  • Various dosage forms: capsule, film-coated tablet, oroflash/chewable tablets, syrup, injection

Interaction

  • Drugs causing Vitamin D3 deficiencies
  • Interaction with Magnesium containing Antacids, Digoxin, Thiazide Diuretics, Cholestyramine, Cholistipol, Phenytoin, Phenobarbital, Orlistat, Mineral Oil, Corticosteroids

Contraindications

  • Contraindicated in diseases associated with hypercalcemia
  • Known hypersensitivity to Vitamin D3 or its class
  • Evidence of Vitamin D3 toxicity

Side Effects

  • Rare symptoms include anorexia, lassitude, nausea & vomiting, diarrhea, constipation, weight loss, polyuria, sweating, headache, thirst, vertigo
  • Raised concentrations of calcium and phosphate in plasma and urine

Pregnancy and Lactation

  • Safe use up to 4000 IU during pregnancy
  • Recommended daily intake for pregnant women: 400 IU
  • Vitamin D3 and its metabolites excreted in breast milk
  • No observed overdose in breastfed infants
  • Prescribing consideration for breast-fed child

Precautions and Warnings

  • Plasma-calcium concentration monitoring for patients on high doses
  • Consultation for patients using Digoxin and Thiazide Diuretics
  • Consultation for patients with liver or kidney disease, primary hyperthyroidism, lymphoma, tuberculosis, and granulomatous disease

Use in Special Populations

  • Safety & efficacy not established in children under 12 years
