Indications

  • Treatment of hypertension
  • Reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Potent vasoconstrictor
  • Primary vasoactive hormone
  • Stimulates aldosterone secretion
  • Selective blocking of angiotensin II to the AT1 receptor
  • Thiazide diuretic

Dosage

  • Usual starting dose of 50/12.5 once daily
  • Dosage may be increased to 100/25 once daily
  • Antihypertensive effect attained within three weeks
  • Severe hypertension starting dose of 50/12.5 once daily
  • Maximum dose is one tablet of 100/25 once daily

Administration

  • May be administered with other antihypertensive agents
  • May be administered with or without food

Interaction

  • No significant drug-drug pharmacokinetic interactions
  • Concomitant use of potassium-sparing diuretics may lead to increase in serum potassium
  • Thiazide diuretics may interact with alcohol, barbiturates, or narcotics

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria or hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Hyperkalaemia
  • Angioedema
  • Vertigo
  • Gastro-intestinal disturbances
  • Angina
  • Palpitation
  • Oedema

Pregnancy & Lactation

  • Avoided in pregnancy unless essential
  • Not recommended in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Hypokalemia may rarely develop
  • Impaired renal function
  • Symptomatic hypotension

Use in Special Populations

  • Use in Patients with Renal Impairment
  • Use in Patients with Hepatic Impairment
  • Safety and effectiveness in pediatric patients have not been established

Overdose Effects

  • Manifestation of overdosage would be hypotension and tachycardia
  • Supportive treatment should be instituted in case of symptomatic hypotension
  • Signs and symptoms observed are those caused by electrolyte depletion
  • Hypokalemia may accentuate cardiac arrhythmias
