Indications
- Treatment of hypertension
- Reducing the risk of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Losartan blocking the vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Hydrochlorothiazide's effect on renal tubular mechanisms of electrolyte reabsorption
Dosage
- Usual starting dose of 50/12.5 is one tablet once daily
- Dosage may be increased to 100/25 once daily for non-responding patients
- Antihypertensive effect attained within three weeks after initiation of therapy
Administration
- May be administered with other antihypertensive agents
- May be administered with or without food
Interaction
- Potential interaction with potassium-sparing diuretics and salt substitutes
- Interaction with antidiabetic drugs and other antihypertensive drugs
Contraindications
- Contraindicated in patients hypersensitive to any component
- Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs
Side Effects
- Symptomatic hypotension including dizziness
- Hyperkalaemia
- Angioedema
- Vertigo
- Gastro-intestinal disturbances
Pregnancy & Lactation
- Angiotensin-II receptor antagonists should be avoided in pregnancy unless essential
- Not recommended in breastfeeding
Precautions & Warnings
- Periodic determination of serum electrolytes
- Hypokalemia may rarely develop
- Impaired renal function
Use in Special Populations
- Usual therapy regimens may be followed in patients with creatinine clearance >30 ml/min
- Not recommended in patients with hepatic impairment
Overdose Effects
- Manifestations of overdosage include hypotension, tachycardia, and bradycardia
- Supportive treatment should be instituted for symptomatic hypotension
