Indications

  • Prevention of postoperative nausea and vomiting (PONV)
  • Prevention of Chemotherapy Induced Nausea and Vomiting (CINV)

Pharmacology

  • Selective high affinity antagonist of human substance P neurokinin 1 (NK1) receptors
  • Prevents substance P from binding to the NK1 receptors

Dosage

  • Recommended oral dosage of Aprepitant for prevention of PONV
  • Regimens for prevention of nausea and vomiting associated with highly and moderately emetogenic cancer chemotherapy

Administration

  • May be taken with or without food
  • No dosage adjustment necessary for elderly patients

Interaction

  • Substrate, weak-to-moderate (dose dependent) inhibitor, and an inducer of CYP3A4
  • Inducer of CYP2C9
  • Precautions while coadministering with drugs using CYP3A4 or CYP2C9
  • Reduces efficacy of hormonal contraceptives

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product
  • Not to be used concurrently with Pimozide, Terfenadine, Astemizole, or Cisapride

Pregnancy & Lactation

  • Pregnancy Category B
  • Decision should be made whether to discontinue nursing or to discontinue the drug based on patient’s importance

Use in Special Populations

  • No dosage adjustment necessary for patients with renal impairment or ESRD undergoing hemodialysis
  • No dosage adjustment necessary for patients with mild to moderate hepatic impairment

Overdose Effects

  • No specific information available on treatment of overdosage
  • Single doses up to 600 mg generally well tolerated in healthy subjects
  • Drowsiness and headache due to overdose
  • Emend should be discontinued in case of overdose
  • General supportive treatment and monitoring should be provided
