Indications
- Prevention of postoperative nausea and vomiting (PONV)
- Prevention of Chemotherapy Induced Nausea and Vomiting (CINV)
Pharmacology
- Selective high affinity antagonist of human substance P neurokinin 1 (NK1) receptors
- Prevents substance P from binding to the NK1 receptors
Dosage
- Recommended oral dosage of Aprepitant for prevention of PONV
- Regimens for prevention of nausea and vomiting associated with highly and moderately emetogenic cancer chemotherapy
Administration
- May be taken with or without food
- No dosage adjustment necessary for elderly patients
Interaction
- Substrate, weak-to-moderate (dose dependent) inhibitor, and an inducer of CYP3A4
- Inducer of CYP2C9
- Precautions while coadministering with drugs using CYP3A4 or CYP2C9
- Reduces efficacy of hormonal contraceptives
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
- Not to be used concurrently with Pimozide, Terfenadine, Astemizole, or Cisapride

Pregnancy & Lactation
- Pregnancy Category B
- Decision should be made whether to discontinue nursing or to discontinue the drug based on patient’s importance
Use in Special Populations
- No dosage adjustment necessary for patients with renal impairment or ESRD undergoing hemodialysis
- No dosage adjustment necessary for patients with mild to moderate hepatic impairment
Overdose Effects
- No specific information available on treatment of overdosage
- Single doses up to 600 mg generally well tolerated in healthy subjects
- Drowsiness and headache due to overdose
- Emend should be discontinued in case of overdose
- General supportive treatment and monitoring should be provided
