Indications

  • Treatment of hypertension
  • Risk reduction of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Blockage of angiotensin II effects
  • Inhibition of aldosterone secretion
  • Reversible competitive inhibition of AT1 receptor
  • Increase in renin activity
  • Reduction in plasma volume
  • Increase in plasma renin activity
  • Increase in aldosterone secretion
  • Increase in urinary potassium loss
  • Decrease in serum potassium
  • Thiazide diuretic effect

Dosage

  • Usual starting dose of 50/12.5 once daily
  • Maximum dose of 100/25 once daily
  • Antihypertensive effect achieved within three weeks
  • No initial dosage adjustment for elderly patients
  • Initial treatment of severe hypertension with 50/12.5 once daily

Administration

  • May be administered with other antihypertensive agents
  • May be administered with or without food

Interaction

  • No significant drug-drug interactions found in studies
  • Potential interaction with potassium-sparing diuretics
  • Interaction with NSAIDs
  • Interaction with antidiabetic drugs
  • Additive effect with other antihypertensive drugs
  • Impaired absorption in the presence of anionic exchange resins

  • Periodic determination of serum electrolytes
  • Hypokalemia may rarely develop
  • Impaired renal function
  • Symptomatic hypotension

Use in Special Populations

  • Usual regimens of therapy followed if creatinine clearance is >30 ml/min
  • Loop diuretics preferred in severe renal impairment
  • Not recommended for titration in hepatic impairment
  • Safety and effectiveness in pediatric patients not established

Overdose Effects

  • Manifestation of hypotension and tachycardia
  • Supportive treatment for symptomatic hypotension
  • No removal by hemodialysis
