Indications

  • Treatment of hypertension
  • Risk reduction of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Losartan blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Maximum dose is 100/25 once daily

Administration

  • May be administered with or without food
  • May be administered with other antihypertensive agents

Interaction

  • No significant drug-drug interactions with specific medications found
  • Potential interactions with antidiabetic drugs and cholestyramine resins

Contraindications

  • Contraindicated in patients hypersensitive to any component
  • Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Occasional occurrence of hyperkalemia
  • Reported cases of angioedema and vertigo

Pregnancy & Lactation

  • Angiotensin-II receptor antagonists should be avoided in pregnancy unless essential
  • Not recommended in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Hypokalemia may rarely develop, especially with brisk diuresis

Use in Special Populations

  • Usual therapy regimens may be followed if creatinine clearance is >30 ml/min
  • Safety and effectiveness in pediatric patients not established

Overdose Effects

  • Manifestations may include hypotension and tachycardia
  • Supportive treatment should be instituted if symptomatic hypotension occurs
