Description

  • Multiple-Unit Pellet System (MUPS)
  • Greater bioavailability
  • Uniform emptying of micro pellets from stomach into small intestine
  • Rapid dissolution of enteric coating and drug release
  • Combines fast acting and sustained action
  • Uniform drug release
  • Once daily dosing
  • Minimizes fluctuation in plasma concentration of drug

Pharmacology

  • Inhibits gastric acid by blocking the hydrogen-potassium adenosine triphosphatase enzyme system

Dosage

  • 40 mg once daily for 4 weeks (adults)
  • 20 mg once daily (maintenance of healing of erosive esophagitis in adults)
  • 20 mg twice daily for 7 days (H. pylori eradication in adults)
  • 40-80 mg twice daily (Zollinger-Ellison syndrome and idiopathic hypersecretion in adults)

Administration

  • Swallowed whole with liquid
  • Can be dispersed in non-carbonated water if necessary

Interaction

  • Extensively metabolized in the liver by CYP2C19 and CYP3A4
  • Not likely to inhibit CYPs 1A2, 2A6, 2C9, 2D6, 2E1 and 3A4
  • Clinically relevant interactions with phenytoin, warfarin, quinidine, clarithromycin, or amoxicillin not expected

Contraindications

  • Known hypersensitivity to any of the formulation

Pregnancy & Lactation

  • Manufacturer advises caution
  • Not known if Esomeprazole or its metabolites appear in human breast milk

Precautions & Warnings

  • Use carefully in patients with severe liver dysfunction and severe renal impairment
  • Slight increase in the risk of hip, wrist, and spine fracture with long-term use
