Indications

  • Treatment of invasive aspergillosis
  • Treatment of candidemia (nonneutropenic)
  • Treatment of disseminated candidiasis
  • Treatment of esophageal candidiasis
  • Treatment of serious infections caused by specific fungi
  • Recommended for patients intolerant of or refractory to other therapy

Pharmacology

  • Mechanism of action involving inhibition of ergosterol synthesis
  • Binding and inhibition of CYP450-dependent 14-alpha sterol demethylase
  • Resulting depletion of ergosterol in fungal cell membrane

Dosage & Administration

  • Oral administration at least one hour before or after a meal
  • Specific loading and maintenance dose regimen based on body weight
  • Injection dosages for different fungal infections

Interaction

  • Adjust dosage and monitor for adverse reactions with CYP3A4, CYP2C9, and CYP2C19 inhibitors and inducers
  • Potential increase in concentrations and activity of CYP3A4, CYP2C9, and CYP2C19 substrate drugs
  • Recommendation to increase maintenance dosage with co-administration of specific drugs

Contraindications

  • Known hypersensitivity to Voriconazole or its components
  • Contraindicated with specific medications due to risk of serious adverse reactions or loss of efficacy

Side Effects

  • Common side effects include abdominal pain, anemia, blurred vision, headache, chest pain, nausea, and diarrhea

Pregnancy & Lactation

  • Lack of adequate and well-controlled studies in pregnant women
  • Use during pregnancy only if potential benefits justify potential risks to the fetus

Precautions & Warnings

  • Long-term exposure (>180 days) requires careful benefit-risk assessment
  • Reported association with squamous cell carcinoma of the skin with long-term treatment
