Pharmacology
Provides anti-inflammatory action by inhibiting Phospholipase A2 enzyme responsible for prostaglandin synthesis; decreases the release of certain chemicals important in the immune system for immunosuppressive action.
Dosage & Administration
- Adults:
- Up to 120 mg/day for acute disorders
- 3-18 mg/day maintenance dose for most conditions
- Children:
- Doses usually lie in the range 0.25-1.5 mg/kg/day
- Juvenile chronic arthritis maintenance dose: 0.25-1.0 mg/kg/day
- Nephrotic syndrome initial dose: usually 1.5 mg/kg/day
- Deflazacort withdrawal: Dose reduction depends on disease relapse likelihood as systemic corticosteroid dose decreases
Interaction
Metabolized in the liver; maintenance dose adjustment required when co-administered with liver enzyme inducers or inhibitors.
Contraindications
Hypersensitivity to any of the ingredients; patients receiving live virus immunization.

Pregnancy & Lactation
- Pregnancy: Corticosteroids may increase the risk of intrauterine growth retardation; benefits should outweigh risks
- Nursing Mother: Excreted in breast milk; doses up to 50 mg daily unlikely to cause systemic effects in the infant
Precautions & Warnings
- Cardiac disease or congestive heart failure: Requires monitoring for salt and water retention, potassium excretion
- Gastritis, esophagitis, diverticulitis, ulcerative colitis: May need special caution if impending perforation, abscess, or infections are probable
- Diabetes mellitus, family history: Special caution required
- Ocular herpes simplex: Caution due to possible corneal perforation
Use in Special Populations
- Hepatic Impairment: Blood levels may be increased; careful monitoring and adjusted minimum effective dose required
- Renal Impairment: No special precautions necessary
- Elderly: No special precautions necessary; common adverse effects may have more serious consequences in old age
