Indications
- Treatment of hypertension
- Reduction of stroke risk in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Affects renal tubular mechanisms of electrolyte reabsorption
Dosage
- Usual starting dose of 50/12.5 is one tablet once daily
- Maximum dose is 100/25 once daily
- Antihypertensive effect is attained within three weeks after initiation of therapy
Administration
- May be administered with other antihypertensive agents
- Can be taken with or without food
Interaction
- No significant drug-drug interactions found in studies with Hydrochlorothiazide, Digoxin, Warfarin, Cimetidine, and Phenobarbital
- Concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium may increase serum potassium
Contraindications
- Contraindicated in patients hypersensitive to any component of the product
- Contraindicated in patients with anuria or hypersensitivity to other sulfonamide-derived drugs
Side Effects
- Symptomatic hypotension including dizziness may occur
- Hyperkalaemia occurs occasionally
- Angioedema has been reported with some angiotensin-II receptor antagonists
Pregnancy & Lactation
- Avoided in pregnancy unless essential
- Not recommended in breastfeeding
- Limited information on use during breastfeeding
Precautions & Warnings
- Periodic determination of serum electrolytes to detect possible imbalance should be performed
- Hypokalemia may rarely develop, especially with brisk diuresis
Use in Special Populations
- Usual therapy regimens may be followed as long as the patient's creatinine clearance is >30 ml/min
- Loop diuretics preferred to thiazides in patients with severe renal impairment
- Not recommended for titration in patients with hepatic impairment
Overdose Effects
- Manifestations of overdosage may include hypotension, tachycardia, and electrolyte depletion
