Indications

  • Treatment of hypertension
  • Risk reduction of stroke in hypertensive patients with left ventricular hypertrophy

Pharmacology

  • Inhibition of angiotensin II binding to AT1 receptor
  • Diuretic effect increasing Sodium and Chloride excretion
  • Reduction of plasma volume and renin activity

Dosage

  • Starting dose of 50/12.5 is one tablet daily
  • Antihypertensive effect attained within three weeks
  • No initial dosage adjustment for elderly patients

Administration

  • Can be administered with or without food
  • May be administered with other antihypertensive agents

Interaction

  • No significant drug-drug interactions with certain medications
  • Potentiation of orthostatic hypotension with alcohol, barbiturates, or narcotics
  • Dosage adjustment of antidiabetic drugs may be required

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product
  • Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Occasional hyperkalaemia
  • Reported occurrences of vertigo, gastrointestinal disturbances, angina, palpitation, oedema, and more

Pregnancy & Lactation

  • Avoided in pregnancy unless essential
  • Not recommended in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes recommended
  • Rare development of hypokalemia
  • Special caution in patients with impaired renal function and symptomatic hypotension

Use in Special Populations

  • Usual therapy regimens for patients with renal impairment if creatinine clearance is >30 ml/min
  • Not recommended for titration in patients with hepatic impairment
  • Safety and effectiveness not established in pediatric patients

Overdose Effects

  • Manifestations include hypotension, tachycardia, and possible bradycardia
  • Supportive treatment recommended for symptomatic hypotension
