Description

  • A combined antihypertensive preparation indicated for the treatment of hypertension and to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy.

Dosage Instructions

  • The usual starting dose is one tablet of 50/12.5 once daily, with the option to increase to 100/25 once daily for patients who do not respond adequately.

Administration

  • May be administered with or without food.

Interaction

  • May interact with potassium-sparing diuretics, non-steroidal anti-inflammatory drugs, antidiabetic drugs, other antihypertensive drugs, cholestyramine and colestipol resins.

Contraindications

  • Contraindicated in patients who are hypersensitive to any component of the product, as well as in patients with anuria or hypersensitivity to other sulfonamide-derived drugs.

Side Effects

  • May include symptomatic hypotension, dizziness, hyperkalemia, vertigo, gastrointestinal disturbances, angina, palpitation, edema, dyspnea, headache, sleep disorders, malaise, urticaria, pruritus, rash, hepatitis, atrial fibrillation, cerebrovascular accident, syncope, pancreatitis, anaphylaxis, cough, depression, erectile dysfunction, anemia, thrombocytopenia, hyponatremia, arthralgia, myalgia, renal impairment, rhabdomyolysis, tinnitus, photosensitivity, vasculitis.

Pregnancy & Lactation

  • Avoid in pregnancy unless essential; not recommended in breastfeeding.

Precautions & Warnings

  • Consider periodic serum electrolyte determination, monitor for hypokalemia, impaired renal function, and symptomatic hypotension.

Special Populations

  • Consider alternative treatment options in pediatric patients, use loop diuretics rather than thiazides in patients with severe renal impairment, not recommended for titration in patients with hepatic impairment.

Overdose Effects

  • Manifestations may include hypotension, tachycardia, bradycardia; supportive treatment should be instituted in case of symptomatic hypotension. Removal by hemodialysis is not effective.

Pharmacology

  • Losartan and its active metabolite block the vasoconstrictor and aldosterone-secreting effects of angiotensin II; Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption, increasing excretion of sodium and chloride.

  • Indicated for the treatment of hypertension and to reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy.

Mode of Action

  • Angiotensin II formed from angiotensin I is a potent vasoconstrictor and stimulates aldosterone secretion; Losartan blocks the binding of angiotensin II to the AT1 receptor; Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption.

Hypertension

  • May require initial dosage of 50/12.5, with options to increase to 100/25 if additional blood pressure reduction is needed.

Severe Hypertension

  • May start with a dose of 50/12.5 once daily, with an option to increase to 100/25 once daily if necessary.
