Indications

  • Treatment of hypertension
  • Reduction of stroke risk in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Potent vasoconstrictor
  • Primary vasoactive hormone of the renin-angiotensin system
  • Stimulates aldosterone secretion
  • Selective blocking of angiotensin II
  • Thiazide diuretic effect
  • Increased excretion of Sodium and Chloride
  • Reduction of plasma volume
  • Increase in plasma renin activity
  • Increase in Aldosterone secretion
  • Increase in urinary Potassium loss
  • Decrease in serum Potassium
  • Mediated by angiotensin II
  • Reversible, competitive inhibitor of the AT1 receptor

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Maximum dose of 100/25 once daily
  • Initial dose for severe hypertension is one tablet of 50/12.5 once daily
  • Initial treatment of severe hypertension is one tablet of 50/12.5 once daily
  • No initial dosage adjustment of 50/12.5 is necessary for elderly patients

Administration

  • May be administered with other antihypertensive agents
  • May be administered with or without food

Interaction

  • No significant drug-drug pharmacokinetic interactions found with Hydrochlorothiazide, Digoxin, Warfarin, Cimetidine, and Phenobarbital
  • Potential interaction with potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium
  • Possible interaction with non-steroidal anti-inflammatory drug Indomethacin
  • Interaction with alcohol, barbiturates, or narcotics-potentiation of orthostatic hypotension may occur
  • Dosage adjustment of the antidiabetic drug may be required with concurrent use
  • Additive effect or potentiation with other antihypertensive drugs
  • Impaired absorption in the presence of anionic exchange resins

Contraindications

  • Hypersensitivity to any component of the product
  • Anuria or hypersensitivity to other sulfonamide-derived drugs

Pregnancy & Lactation

Precautions & Warnings

  • Hypersensitivity: Angiooedema
  • Periodic determination of serum electrolytes
  • Hypokalemia risk

Overdose Effects

  • Manifestations of overdosage including hypotension, tachycardia, and bradycardia
  • Supportive treatment for symptomatic hypotension
  • Potential electrolyte depletion and dehydration from excessive diuresis
  • Possible accentuation of cardiac arrhythmias if digitalis has been administered
  • Removal by hemodialysis not established
