Medicine Details

Indications

  • Treatment of hypertension
  • Risk reduction of stroke in hypertensive patients with left ventricular hypertrophy

Pharmacology

  • Block vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Selective binding blockade of angiotensin II to the AT1 receptor
  • Thiazide diuretic effect on renal tubular mechanisms of electrolyte reabsorption
  • Increase in excretion of Sodium and Chloride
  • Reduction in plasma volume and increase in plasma renin activity

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Maximum dose is 100/25 once daily
  • Antihypertensive effect is attained within three weeks after initiation of therapy

Administration

  • May be administered with or without food
  • May be administered with other antihypertensive agents

Interaction

  • No significant drug-drug interactions found in interaction studies with various drugs
  • Concomitant use of potassium-sparing diuretics may lead to increase in serum potassium
  • Potential interaction with alcohol, barbiturates, narcotics, and antidiabetic drugs

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product
  • Contraindicated in patients with anuria or hypersensitivity to sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Occasional hyperkalaemia
  • Reports of angioedema, vertigo, gastro-intestinal disturbances, and headache
  • Rare reports of hepatits, atrial fibrillation, and renal impairment

Pregnancy & Lactation

  • Angiotensin-II receptor antagonists should be avoided in pregnancy unless essential
  • Limited information on the use of angiotensin-II receptor antagonists in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Rare development of hypokalemia
  • Caution in patients with impaired renal function and symptomatic hypotension

Use in Special Populations

  • Use caution with renal impairment
  • Not recommended for titration in patients with hepatic impairment
  • Safety and effectiveness in pediatric patients not established

Overdose Effects

  • Manifestations of hypotension and tachycardia in overdosage
  • Supportive treatment should be instituted in case of symptomatic hypotension
  • Limited data available on overdosage in humans
