Indications

  • Treatment of hypertension
  • Reducing risk of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Block vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Reversible, competitive inhibitor of the AT1 receptor
  • Increase excretion of Sodium and Chloride
  • Reduce plasma volume
  • Increases in plasma renin activity
  • Increases in Aldosterone secretion
  • Increases in urinary Potassium loss
  • Decreases in serum Potassium

Dosage

  • Usual starting dose of 50/12.5 is one tablet once daily
  • Maximum dose is 100/25 once daily
  • Initial dose in hypertensive patients with left ventricular hypertrophy is 50/12.5
  • Maximum dose for severe hypertension is 100/25 once daily
  • Antihypertensive effect is attained within three weeks after initiation of therapy
  • No initial dosage adjustment for elderly patients
  • Maximum dose of 100/25 should not be used as initial therapy in elderly patients

Administration

May be administered with or without food

Interaction

  • No significant drug-drug interactions with Losartan Potassium
  • Interaction with Thiazide diuretics and antidiabetic drugs
  • Additive effect or potentiation with other antihypertensive drugs
  • Impaired absorption in the presence of anionic exchange resins

Contraindications

Hypersensitivity to any component of the product, anuria, hypersensitivity to sulfonamide-derived drugs

Pregnancy & Lactation

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Hypokalemia may rarely develop
  • Impaired renal function
  • Hypersensitivity reactions
  • Symptomatic hypotension

Use in Special Populations

  • Usual regimens of therapy with 50/12.5 may be followed if patient's creatinine clearance is >30 ml/min
  • Not recommended for titration in patients with hepatic impairment
  • Safety and effectiveness not established in pediatric patients

Overdose Effects

  • Manifestations of overdosage could be hypotension, tachycardia, bradycardia
  • Supportive treatment should be instituted if symptomatic hypotension occurs
