Indications

  • Treatment of hypertension
  • Reducing risk of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Thiazide diuretic affecting renal tubular mechanisms of electrolyte reabsorption

Dosage

  • Starting dose of 50/12.5 once daily
  • Increase to 100/25 once daily if needed
  • Antihypertensive effect attained within three weeks
  • No initial dosage adjustment for elderly patients

Administration

  • May be administered with other antihypertensive agents
  • Can be administered with or without food

Interaction

  • No significant drug-drug interactions found in studies
  • Potential interactions with potassium-sparing diuretics
  • Interaction with antidiabetic drugs
  • Additive effect with other antihypertensive drugs

Contraindications

  • Hypersensitivity to any component
  • Anuria or hypersensitivity to other sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Occasional hyperkalaemia
  • Reports of angioedema
  • Possible vertigo and gastrointestinal disturbances

Pregnancy & Lactation

  • Avoided in pregnancy unless essential
  • Not recommended in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Risk of hypokalemia
  • Impaired renal function
  • Symptomatic hypotension

Use in Special Populations

  • Usual regimens of therapy in patients with renal impairment
  • Not recommended in patients with severe hepatic impairment
  • Safety and effectiveness not established in pediatric patients

Overdose Effects

  • Manifestations include hypotension and tachycardia
  • Signs and symptoms caused by electrolyte depletion
  • Supportive treatment in case of symptomatic hypotension
