Indications
- Treatment of hypertension
- Reduce the risk of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Primary vasoactive hormone of the renin-angiotensin system
- Potent vasoconstrictor
- Stimulates aldosterone secretion
- Selective blocker of AT1 receptor
- Inhibits ACE
- Thiazide diuretic
- Affects renal tubular mechanisms of electrolyte reabsorption
- Reduces plasma volume
- Increases plasma renin activity
- Increases aldosterone secretion
- Increases urinary potassium loss
- Decreases serum potassium
Dosage
- Usual starting dose of 50/12.5 is one tablet once daily
- Maximum dose is 100/25 once daily
- Antihypertensive effect attained within three weeks after initiation of therapy
- No initial dosage adjustment necessary for elderly patients
Administration
- May be administered with other antihypertensive agents
- May be administered with or without food
Interaction
- No significant drug-drug pharmacokinetic interactions found in interaction studies with various drugs
- Concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium may lead to increase in serum potassium
- Potential interactions with alcohol, barbiturates, narcotics, antidiabetic drugs, other antihypertensive drugs, cholestyramine, and colestipol resins

- Hypersensitivity to any component of the product
- Anuria
- Hypersensitivity to sulfonamide-derived drugs

- Use in Patients with Renal Impairment
- Use in Patients with Hepatic Impairment
- Use in pediatric patients
- Not recommended for titration in patients with hepatic impairment
Use in Special Populations
- Usual regimens of therapy with 50/12.5 may be followed with creatinine clearance >30 ml/min
- Loop diuretics preferred to thiazides in more severe renal impairment
- Safety and effectiveness in pediatric patients not established
Overdose Effects
- Manifestations of hypotension, tachycardia, bradycardia
- Supportive treatment for symptomatic hypotension
- Limited data available in regard to overdosage in humans
- Signs and symptoms caused by electrolyte depletion
