Indications
- Treatment of hypertension
- Reducing risk of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Losartan blocking vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Hydrochlorothiazide affecting renal tubular mechanisms of electrolyte reabsorption
Dosage
- Starting dose of 50/12.5 is one tablet once daily
- Dosage may be increased to 100/25 once daily for non-responsive patients
- Antihypertensive effect attained within three weeks
- No initial dosage adjustment needed for elderly patients
- Maximum dose of 100/25 should not be used as initial therapy in elderly patients
- Starting dose for severe hypertension is one tablet of 50/12.5 once daily
- Maximum dose for severe hypertension is one tablet of 100/25 once daily
Administration
- May be administered with other antihypertensive agents
- May be administered with or without food
Interaction
- Concomitant use of potassium-sparing diuretics, potassium supplements, or salt substitutes containing potassium may lead to increase in serum potassium
- Antihypertensive effect may be blunted by non-steroidal anti-inflammatory drug Indomethacin
- Alcohol, barbiturates, or narcotics may potentiate orthostatic hypotension when administered concurrently with thiazide diuretics
- Dosage adjustment of antidiabetic drugs may be required when administered concurrently with Hydrochlorothiazide
- Additive effect or potentiation may occur when other antihypertensive drugs are used concurrently with Pertilos Plus
- Absorption of Hydrochlorothiazide is impaired in the presence of anionic exchange resins like cholestyramine and colestipol resins
Contraindications
- Hypersensitivity to any component of the product
- Anuria or hypersensitivity to other sulfonamide-derived drugs

Pregnancy & Lactation
- Angiotensin-II receptor antagonists should be avoided in pregnancy unless essential
- Not recommended in breastfeeding
- Alternative treatment options with better-established safety information are available during breastfeeding
Precautions & Warnings
- Periodic determination of serum electrolytes
- Hypokalemia may rarely develop
- Impaired renal function
- Symptomatic hypotension
Use in Special Populations
- Usual regimens of therapy with 50/12.5 may be followed as long as the patient's creatinine clearance is >30 ml/min
- Loop diuretics preferred to thiazides in patients with more severe renal impairment
- Combination not recommended for titration in patients with hepatic impairment
- Safety and effectiveness in pediatric patients not established
Overdose Effects
- Manifestation of overdosage would be hypotension and tachycardia
- Supportive treatment should be instituted in case of symptomatic hypotension
- Degree to which Hydrochlorothiazide is removed by hemodialysis has not been established
