Indications

  • Treatment of hypertension
  • Reducing the risk of stroke in patients with hypertension and left ventricular hypertrophy

Pharmacology

  • Losartan blocking the vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Hydrochlorothiazide affecting renal tubular mechanisms of electrolyte reabsorption

Dosage

  • Starting dose of 50/12.5 is one tablet once daily
  • Antihypertensive effect attained within three weeks after initiation of therapy

Administration

  • May be administered with other antihypertensive agents
  • May be administered with or without food

Interaction

  • No significant drug-drug pharmacokinetic interactions found
  • Concomitant use of potassium-sparing diuretics may lead to increase in serum potassium

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product
  • Contraindicated in patients with anuria or hypersensitivity to other sulfonamide-derived drugs

Side Effects

  • Symptomatic hypotension including dizziness
  • Hyperkalaemia occurs occasionally
  • Angioedema has been reported with some angiotensin-II receptor antagonists

Pregnancy & Lactation

  • Angiotensin-II receptor antagonists should be avoided in pregnancy unless essential
  • Not recommended in breastfeeding

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Hypokalemia may rarely develop

Use in Special Populations

  • Use in Patients with More Severe Renal Impairment
  • Use in Patients with Hepatic Impairment not recommended for titration
  • Safety and effectiveness in pediatric patients not established

Overdose Effects

  • Symptomatic hypotension should occur, supportive treatment should be instituted
  • Digitalis administered, hypokalemia may accentuate cardiac arrhythmias
