Indications
- Treatment of hypertension
- Risk reduction of stroke in patients with hypertension and left ventricular hypertrophy
Pharmacology
- Block vasoconstrictor and aldosterone-secreting effects of angiotensin II
- Thiazide diuretic effect on renal tubular mechanisms of electrolyte reabsorption
Dosage
- Usual starting dose of 50/12.5 is one tablet once daily
- Maximum dose of 100/25 once daily for severe hypertension
- Antihypertensive effect attained within three weeks after initiation of therapy
Administration
- May be administered with other antihypertensive agents
- May be administered with or without food
Interaction
- No significant drug-drug pharmacokinetic interactions found
- Potential interactions with antidiabetic drugs, other antihypertensive drugs, and cholestyramine and colestipol resins
Contraindications
- Hypersensitivity to any component of the product
- Anuria or hypersensitivity to other sulfonamide-derived drugs
Side Effects
- Symptomatic hypotension including dizziness
- Occasional hyperkalaemia
- Vertigo, gastrointestinal disturbances, angina, palpitation, oedema, dyspnoea, headache, sleep disorders, malaise, urticaria, pruritus, rash
- Rarely hepatitis, atrial fibrillation, cerebrovascular accident, syncope, paraesthesia, pancreatitis, anaphylaxis, cough, depression, erectile dysfunction, etc.
Pregnancy & Lactation
- Avoided in pregnancy unless essential
- Not recommended in breastfeeding
Precautions & Warnings
- Periodic determination of serum electrolytes
- Hypokalemia may rarely develop
- Impaired renal function and symptomatic hypotension
Use in Special Populations
- Usual regimens of therapy in renal impairment
- Not recommended for titration in hepatic impairment
- Safety and effectiveness not established in pediatric patients
Overdose Effects
- Manifestations of hypotension and tachycardia
- Signs and symptoms caused by electrolyte depletion
- Supportive treatment required if symptomatic hypotension occurs
- Degree of removal by hemodialysis not established
