Indications

• Treatment of hypertension

Pharmacology

• Olmesartan is a reversible & competitive inhibitor of AT 1 receptor
• Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption

Dosage & Administration

• Usual starting dose of 20/12.5 mg once daily
• May be titrated to two tablets 40/25 mg once daily based on blood pressure response

Interaction

• Olmesartan has no significant drug interactions
• Hydrochlorothiazide may interact with alcohol, barbiturates, narcotics, antidiabetic drugs, other antihypertensive drugs, corticosteroids, ACTH, and lithium

Contraindications

• Hypersensitivity to any component of the product
• Anuria or hypersensitivity to other sulfonamide-derived drugs

• Upper respiratory tract infection
• Urinary tract infection
• Chest pain
• Back pain
• Peripheral edema
• Abdominal pain
• Dyspepsia
• Gastroenteritis
• Diarrhea

Pregnancy & Lactation

• Safety and effectiveness not established, should be discontinued during pregnancy and lactation

Precautions & Warnings

• Periodic determination of serum electrolytes should be performed
• Hyperuricemia may occur in certain patients receiving thiazide therapy
• Impaired renal function

Use in Special Populations

• Renal impairment patients - therapy may be followed if creatinine clearance is >30 ml/min
• Hepatic impairment - no dosage adjustment necessary
• Paediatric use - safety and effectiveness not established
• Geriatric use - cautious dose selection for elderly patients

Overdose Effects

• Olmesartan - hypotension and tachycardia
• Hydrochlorothiazide - electrolyte depletion, hypokalemia, hypochloremia, dehydration
