Indications

  • Treatment of hypertension
  • Can be used alone or in combination with other antihypertensive agents

Pharmacology

  • Blocks vasoconstrictor effects of angiotensin II
  • Selectively blocks the binding of angiotensin II to the AT 1 receptor in vascular smooth muscle
  • Has more than a 12,500-fold greater affinity for the AT 1 receptor than for the AT 2 receptor

Dosage & Administration

  • Usual recommended starting dose of 20 mg once daily
  • May be increased to 40 mg for further blood pressure reduction after 2 weeks
  • Twice-daily dosing offers no advantage over once daily
  • No initial dosage adjustment for elderly patients
  • May be administered with or without food
  • No established safety and effectiveness in pediatric patients

Interaction

  • No significant drug interactions reported with digoxin, warfarin, or antacids
  • Not metabolized by the cytochrome P450 system
  • Potential deterioration of renal function with NSAIDs including selective COX-2 inhibitors

Contraindications

  • Contraindicated in patients hypersensitive to any component of the product

Side Effects

  • Well tolerated with an incidence of adverse reactions similar to placebo
  • Most common adverse reaction was dizziness (3% vs. 1%)
  • Other adverse reactions occurred at an incidence of more than 1% but also occurred at similar or greater incidence in patients receiving placebo

Pregnancy & Lactation

  • Discontinue in pregnancy as soon as possible
  • Can cause injury and death to the developing fetus
  • Not known whether excreted in human milk
  • Consideration should be made whether to discontinue nursing or discontinue the drug

Precautions & Warnings

  • Changes in renal function may be anticipated
  • Treatment has been associated with oliguria, progressive azotemia, acute renal failure, and death in susceptible individuals

Overdose Effects

  • No experience of overdose reported
  • Most likely effects of overdosage are hypotension and tachycardia
  • Clinically significant hypotension requires active support of the cardiovascular system
