Indications

  • spasticity resulting from multiple sclerosis
  • flexor spasms and concomitant pain
  • skeletal muscle spasm resulting from rheumatic disorders
  • spinal cord injuries and other spinal cord diseases
  • cerebrovascular accidents or neoplastic or degenerative brain disease

Pharmacology

  • Baclofen inhibits monosynaptic and polysynaptic reflexes
  • Stimulates GABA receptors
  • Inhibits release of glutamate and aspartate
  • Produces CNS depression
  • Exerts antinociceptive effect

Dosage & Administration

  • Adult & children over 10 years: 5 mg 3 times daily
  • Preferably with or after food
  • Gradually increased
  • Max. 100 mg daily
  • Children <10 years: Treatment usually started with 2.5 mg given 4 times daily
  • Raised according to requirement
  • Daily maintenance dose based on age range

Interaction

  • Increased sedation with agents acting on the central nervous system
  • Increased fall in blood pressure with antihypertensives
  • Potentiation of pharmacological effects with tricyclic antidepressants
  • Reports of mental confusion, hallucinations, headaches, nausea and agitation with levodopa and carbidopa
  • Increased CNS depressant effects with MAO inhibitors
  • Caution with magnesium sulphate or other neuromuscular blocking agents

Contraindications

  • Hypersensitivity to any component of the product

  • Transient drowsiness
  • Daytime sedation
  • Dizziness
  • Weakness
  • Fatigue
  • Common central nervous system reactions including headache, insomnia, euphoria, depression, confusion
  • Cardiovascular reactions like hypotension
  • Gastrointestinal reactions including nausea, constipation
  • Genitourinary reactions like urinary frequency
  • Other reactions like rash, excessive perspiration, weight gain

Pregnancy & Lactation

  • Pregnancy category B3
  • Safe use not established
  • Crosses placental barrier
  • Should only be administered when potential benefits outweigh possible hazards
  • Excreted in breast milk in small quantities

Precautions & Warnings

  • Lower doses for impaired renal function or chronic haemodialysis
  • Cautious treatment for patients with psychotic disorders, schizophrenia, depressive or manic disorders
  • Use with supervision in patients with epilepsy, muscle spasticity, and adequate anticonvulsive therapy
  • Caution in patients with peptic ulcers, cerebrovascular diseases, or hepatic, renal or respiratory failure
  • Monitoring of respiratory and cardiovascular function
  • Caution in patients with neurogenic disturbances affecting bladder emptying
  • Not significantly beneficial for stroke patients
  • Appropriate laboratory tests for patients with hepatic diseases or diabetes mellitus

Overdose Effects

  • Importance of gastric lavage in case of severe overdose
