Description
- Diastat is a sodium glucose co-transporter-2 (SGLT-2) inhibitor used to improve glycemic control in adults with type 2 diabetes mellitus.
DosageAndAdministration
- The recommended dose is 10 mg once daily, taken in the morning, with or without food. The dose may be increased to 25 mg once daily in patients tolerating the initial dose.
Pharmacology
- Empagliflozin inhibits SGLT2 co-transporters, leading to increased glucose excretion, reduced hyperglycemia, weight loss, and lowered blood pressure.
Indications
- Adjunct to diet and exercise for glycemic control in adults with type 2 diabetes mellitus, and to reduce the risk of cardiovascular death in adult patients with type 2 diabetes mellitus and established cardiovascular disease.
Interaction
- Increased urine volume when co-administered with diuretics
- Increased risk for hypoglycemia when co-administered with insulin or insulin secretagogues
- Inaccurate results in urine glucose tests and interference with 1,5-AG assay when using SGLT2 inhibitors
Contraindications
- History of serious hypersensitivity reaction to Empagliflozin or its ingredients
- Severe renal impairment, end-stage renal disease, or dialysis

PrecautionsAndWarnings
- Assess renal function prior to initiation and periodically thereafter. Do not initiate in patients with an eGFR less than 45 ml/min/1.73 m2. No dose adjustment needed in patients with an eGFR greater than or equal to 45 ml/min/1.73 m2.
OverdoseEffects
- Usual supportive measures should be employed. Hemodialysis study not conducted.
