Indications

  • Treatment of hypertension

Pharmacology

  • Inhibition of angiotensin-II effects
  • Aldosterone secretion inhibition
  • Diuretic action on renal tubular mechanisms
  • Increase in Sodium and Chloride excretion
  • Reduction in plasma volume
  • Increase in plasma renin activity
  • Increase in Aldosterone secretion
  • Urinary Potassium loss
  • Decrease in serum Potassium

Dosage & Administration

  • Usual starting dose of 20/12.5 mg
  • Individualized dosing
  • Titration based on blood pressure response
  • Maximum dose of two tablets 40/25 mg once daily

Interaction

  • No significant drug interactions with Olmesartan Medoxomil
  • Potential interactions with Thiazide diuretics and: Alcohol, Barbiturates, Narcotics, Antidiabetic drugs, Other antihypertensive drugs, Corticosteroids, ACTH, Lithium

Contraindications

  • Hypersensitivity to any component
  • Anuria
  • Hypersensitivity to sulfonamide-derived drugs

Precautions & Warnings

  • Periodic determination of serum electrolytes
  • Detection of electrolyte imbalance like hypokalemia, hyponatremia, and hypochloremic alkalosis
  • Possible occurrence of hyperuricemia
  • Impaired renal function

Use in Special Populations

  • Suitable for renal impairment patients with creatinine clearance >30 ml/min
  • Loop diuretics preferred for severe renal impairment
  • No dosage adjustment needed with hepatic impairment
  • Not established for pediatric use
  • Caution in dose selection for geriatric use

Overdose Effects

  • Manifestations of Olmesartan overdosage: hypotension and tachycardia
  • Manifestations of Hydrochlorothiazide overdosage: electrolyte depletion, hypokalemia, hypochloremia, dehydration
