Indications
- Treatment of mild to moderate hypertension in patients who have been stabilised on the individual components given in the same proportion
Pharmacology
- Ramipril is an angiotensin converting enzyme (ACE) inhibitor
- Reduces plasma angiotensin II
- Decreases vasopressor activity
- Decreases aldosterone secretion
- Effective in managing heart failure
- Reduces risk of stroke, myocardial infarction, and death from cardiovascular events
- Long-acting and well-tolerated
- Hydrochlorothiazide is a thiazide diuretic
- Directly increases excretion of sodium and chloride
- Reduces plasma volume
- Increases plasma renin activity
- Increases aldosterone secretion
- Increases urinary potassium loss
- Decreases serum potassium
Dosage & Administration
- Dose titration guided by clinical effect
- Maximum daily dose: 10 mg ramipril and 50 mg hydrochlorothiazide
- Convenience for patients receiving ramipril and hydrochlorothiazide from separate tablets
Interaction
- Potentiation with diuretics or other antihypertensive agents
- Weakening of the effectiveness of blood sugar lowering medications
- Potentiation with alcohol
Contraindications
- Hypersensitivity to ramipril, hydrochlorothiazide, or other thiazide diuretics
- History of hereditary angioneurotic edema
- Severe impairment of renal function
- Haemodynamically relevant renal artery stenosis
- Low blood pressure or unstable circulatory situation
- Clinically relevant electrolyte disturbances

Pregnancy & Lactation

- Potential for serious adverse reactions in nursing infants
Precautions & Warnings
- Require regular medical supervision
- Correction of dehydration, reduced blood volume, or salt depletion before initiating treatment
- Special caution in patients with severe and malignant hypertension
- Monitoring of white blood cell count
- Insufficient experience in use for children
Use in Special Populations
- Dosage adjustment in renal impairment
- Safety and effectiveness not established in pediatric patients
Overdose Effects
- Treatment of Ramipril overdosage with infusion of normal saline solution
- Caution in case of electrolyte depletion and excessive diuresis
- Unknown removal of Hydrochlorothiazide by hemodialysis
