Indications
- Chronic management of hyperuricemia in gout patients
Pharmacology
- Non-purine xanthine oxidase (XO) inhibitor
- Decreases serum uric acid level
- Inhibits xanthine oxidase
- Does not inhibit other enzymes involved in purine and pyrimidine synthesis
Dosage & Administration
- Recommended starting dose of Febuxostat 40mg tablet once daily
- Adjustment of dosage based on serum uric acid levels
- Management of gout flares with NSAID or colchicine
- No dose adjustment necessary for renal or mild to moderate hepatic impairment
Interaction
- Not recommended to be used with mercaptopurine or azathioprine
- No dose adjustment necessary for co-administration with rosiglitazone or CYP2C8 substrates
- Inhibition by NSAIDs and probenecid may affect elimination
- Potent inducers of UGT enzymes may affect metabolism
- No dose adjustment necessary for colchicine, indomethacin, hydrochlorothiazide, or warfarin
- Antacid ingestion may cause delayed absorption
Contraindications
- Contraindicated in patients treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Common adverse reactions include gout flares, liver function abnormalities, diarrhea, nausea, headache, rash, and edema
- Rare serious hypersensitivity reactions observed
Pregnancy & Lactation
- Pregnancy Category C
- Use during pregnancy only if potential benefit justifies potential risk
- Caution when administered to nursing women
Precautions & Warnings
- Increased gout flares observed during initiation
- Higher rate of cardiovascular thromboembolic events observed
- Monitor signs and symptoms of MI and stroke
- Transaminase elevations observed, monitor liver function tests
Overdose Effects
- Studied in healthy subjects up to 300mg daily for 7 days without evidence of dose-limiting toxicities
