Indications
- Chronic management of hyperuricemia in patients with gout
Pharmacology
- Febuxostat is a non-purine, selective xanthine oxidase (XO) inhibitor
- Decreases serum uric acid level by inhibiting xanthine oxidase
- Not expected to inhibit other enzymes involved in purine and pyrimidine synthesis
Dosage & Administration
- Recommended starting dose of Febuxostat 40 mg tablet once daily
- Dose adjustment for patients who do not achieve target serum uric acid levels
- Management of tumor lysis syndrome with specific dosage
- Prophylaxis for gout flares
- No dose adjustment required for renal and hepatic impairment
- Safety and effectiveness not established for pediatric use
Interaction
- Contraindicated with mercaptopurine, azathioprine, or theophylline
- No dose adjustment required for co-administration with rosiglitazone or CYP2C8 substrates
- Effect on metabolism when co-administered with naproxen and other inhibitors of glucuronidation
- Possible impact of inducers of glucuronidation on efficacy
- Co-administration with colchicine, indometacin, hydrochlorothiazide, warfarin, and antacids
Contraindications
- Contraindicated in patients being treated with azathioprine, mercaptopurine, or theophylline
Side Effects
- Most commonly reported adverse reactions include gout flares, liver function abnormalities, diarrhoea, nausea, headache, rash, and edema
- Rare serious hypersensitivity reactions observed
Pregnancy & Lactation
- Pregnancy Category C
- Use only if potential benefit justifies potential risk to the fetus
- Caution advised during lactation
Precautions & Warnings
- Increase in gout flares during initiation of treatment
- Higher rate of cardiovascular thromboembolic events observed
- Transaminase elevations observed, monitor liver function tests
Overdose Effects
- Studied in healthy subjects in doses up to 300 mg daily for seven days without evidence of dose-limiting toxicities
