Indications

  • Treatment of hypertension
  • Lowering blood pressure
  • Reducing the risk of cardiovascular events
  • Can be used alone or with other antihypertensive agents

Pharmacology

  • Telmisartan blocks the vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Blocks the binding of angiotensin II to the AT1 receptor
  • Does not inhibit ACE (kininase II)
  • Hydrochlorothiazide affects renal tubular mechanisms of electrolyte reabsorption

Dosage & Administration

  • Initial dose: Telmisartan monotherapy 80 mg or Hydrochlorothiazide 25 mg once daily
  • Can be titrated up to Telmisartan and Hydrochlorothiazide 160 mg/25 mg after 2 to 4 weeks
  • May be administered with other antihypertensive drugs

Interaction

  • Caution with aliskiren, digoxin, lithium, other high blood pressure medicines
  • Potential interference with certain laboratory tests

Contraindications

  • Hypersensitivity to Telmisartan, Hydrochlorothiazide, or any other component
  • Do not co-administer with Aliskiren in diabetic patients

  • Not recommended in severe renal impairment

Overdose Effects

  • Manifestations of overdosage: hypertension, dizziness, tachycardia
  • Not removed by hemodialysis
