Indications

  • Treatment of hypertension
  • Lowering blood pressure
  • Reducing risk of cardiovascular events
  • Use alone or with other antihypertensive agents

Pharmacology

  • Telmisartan: Blocks vasoconstrictor and aldosteronesecreting effects
  • Telmisartan: Blocks binding of angiotensin II to AT1 receptor
  • Telmisartan: Greater affinity for AT1 receptor than AT2 receptor
  • Hydrochlorothiazide: Affects renal tubular mechanisms of electrolyte reabsorption

Dosage & Administration

  • Initial dose of Telmisartan monotherapy 80 mg
  • Titration to 160 mg/25 mg after 2 to 4 weeks if necessary
  • May be administered with other antihypertensive drugs

Interaction

  • Caution with aliskiren, digoxin, lithium, other high blood pressure medicines
  • Caution with NSAIDs, corticosteroids, ACE blockers, angiotensin II receptor blockers
  • Potential interference with certain laboratory tests

Contraindications

  • Hypersensitivity to Telmisartan, Hydrochlorothiazide or any other component
  • Do not co-administer Aliskiren with this tablet in patients with Diabetes

Pregnancy & Lactation

  • Caution in patients with kidney disease, liver disease, dehydration, gout, lupus
  • Regular blood sugar checks for diabetic patients
  • Caution with potassium supplements or salt substitutes
  • May cause dizziness

Use in Special Populations

  • Safety and effectiveness not established in pediatric patients
  • Cautious dose selection for elderly patients
  • Initiate treatment under close medical supervision in patients with hepatic impairment

Overdose Effects

  • Manifestations include hypertension, dizziness, tachycardia
  • Manifestations include hypokalemia, hyponatremia, dehydration
  • Telmisartan not removed by hemodialysis
