Pharmacology
- Contains Amlodipine and Bisoprolol Fumarate
- Amlodipine is a dihydropyridine calcium antagonist
- Bisoprolol Fumarate is a beta1-selective adrenoceptor blocking agent
Dosage & Administration
- One tablet once daily
Interactions
- Combinations not recommended with calcium antagonists of the verapamil and diltiazem type, centrally-acting antihypertensive drugs
- Combinations to be used with caution with strong or moderate CYP3A4 inhibitors, CYP3A4 inducers, simvastatin, Tacrolimus, Cyclosporine, class I antiarrhythmic drugs, class III antiarrhythmic drugs, parasympathomimetic drugs, topical beta-blockers, insulin and oral antidiabetic drugs, anesthetic agents, digitalis glycosides, NSAIDs, sympathomimetic agents, antihypertensive agents, Mefloquine, Rifampicin, Ergotamine derivatives, MAO inhibitors
Contraindications
- Acute heart failure or during episodes of heart failure decompensation
- Obstruction of the outflow tract of the left ventricle
- Cardiogenic shock
- Second or third degree AV block
- Sick sinus syndrome
- Symptomatic bradycardia or hypotension
- Severe bronchial asthma
- Severe forms of peripheral arterial occlusive disease or severe forms of Raynaud’s syndrome
- Untreated phaeochromocytoma metabolic acidosis
- Hypersensitivity to bisoprolol, amlodipine, dihydropyridine derivates or to any of the excipients
Side Effects
- Common: Dizziness, headache, somnolence, palpitations, flushing, gastrointestinal complaints, edema, fatigue
- Uncommon: Insomnia, mood changes, paresthesia, visual disturbances, bradycardia, hypotension, syncope, dyspnea, bronchospasm, skin discoloration
- Rare: Allergic reactions, nightmares, hallucinations, confusion, increased triglycerides, increased liver enzymes
Pregnancy & Lactation
- Not recommended
Precautions & Warnings
- Caution in patients with heart failure
- Caution in patients with impaired hepatic function
- Avoid in patients with obstructive airways diseases
- Caution in bronchospasm, diabetes mellitus, first degree AV block, Prinzmetal’s angina
- Careful use in patients with psoriasis or history of psoriasis
- Caution in patients undergoing general anesthesia
Use in Special Populations
- Caution in geriatric use
- Safety and efficacy not established in pediatric use
- Caution in patients with liver disease
- No dosage adjustment required for patients with mild to moderate renal impairment
- Caution in patients undergoing dialysis
Overdose Effects
- Expected signs of overdose include bradycardia, hypotension, bronchospasm, acute cardiac insufficiency, hypoglycemia
- Gross overdose of amlodipine could result in excessive peripheral vasodilation and reflex tachycardia
