Indications
- Treatment of hypertension
Pharmacology
- Contains Valsartan, an angiotensin receptor blocker
- Contains Hydrochlorothiazide, a thiazide diuretic
- Blocks angiotensin II receptor
- Dilates blood vessels and reduces blood pressure
- Specific angiotensin II antagonist
Dosage & Administration
- Combination therapy for uncontrolled hypertension
- Maximum dose of Valsartan and Hydrochlorothiazide
- No initial dosage adjustment for elderly
- Not established for pediatric use
Interaction
- Potential interactions with diuretics
- Potential interaction with agents increasing serum potassium
- Potential interaction with lithium salts
- Possible interactions with alcohol, barbiturates, narcotics
- May affect absorption of other drugs such as cholestyramine and colestipol resins
Contraindications
- Hypersensitivity to any component of the product
- Anuria
- Hypersensitivity to sulfonamide-derived drugs
Side Effects
- Rare side effects
- Common side effects include headache, dizziness, fatigue, abdominal pain, cough, diarrhea, nausea
- Potential side effects such as hyperkalemia, impotency, reduced renal function, allergic reactions, dyspnea, constipation, back pain, muscle cramps, rash, anxiety, insomnia, vertigo, hypotension
Pregnancy & Lactation
- Not recommended for use in pregnancy
- Not known if excreted in human milk
Precautions & Warnings
- Exercise caution in patients with impaired hepatic function
- Exercise caution in patients with impaired renal function
- Caution in initiating therapy in patients with heart failure and post-myocardial infarction
Use in Special Populations
- Usual therapy regimens for patients with creatinine clearance >30 ml/min
- Loop diuretics preferred for patients with severe renal impairment
- Care needed in patients with hepatic impairment
Overdose Effects
- Manifestations of overdosage include hypotension, tachycardia, bradycardia
- Management of Valsartan overdose with supine position and intravenous infusion of normal saline
- Management of Hydrochlorothiazide overdose for electrolyte depletion and potential cardiac arrhythmias
