Indications
- Short term management of bleeding oesophageal varices
- Emergency treatment of type 1 hepatorenal syndrome
Dosage & Administration
- Initial i.v. injection of 2 mg terlipressin acetate every 4 hours
- Adjustable dosage based on body weight and adverse effects
- 3-4 mg terlipressin acetate i.v. every 24 hours for type 1 hepatorenal syndrome
- Standard average duration of treatment: 10 days
Interaction
- Increased hypotensive effect with non-selective beta-blockers
- Lowered heart rate and cardiac output with concomitant bradycardiac effect medications
- Extreme caution with medications that can prolong the QT interval
- Potential triggering of 'torsade de pointes'
- Caution with medications causing hypokalaemia or hypomagnesemia
Contraindications
- Hypersensitivity to the active substance or excipients
- Contraindicated in pregnancy

Pregnancy & Lactation
- Contraindicated during pregnancy
- Potential harmful effects on pregnancy and foetus
- Unknown excretion in human breast milk

Precautions & Warnings
- Regular monitoring and control of various parameters during treatment
- Caution in patients with cardiac, pulmonary, or vascular disease
- Not recommended for use in septic shock with low cardiac output
- Avoid local necrosis with intravenous administration
- Caution with predisposing factors for QT interval prolongation and ventricular arrhythmias
- Ascertain acute functional renal failure prior to use for hepatorenal syndrome
- Exercise caution in paediatric and elderly patients
- Contains 1.33 mmol of sodium per ampoule
Use in Special Populations
- No recommended dosage available for elderly or paediatric patients
Overdose Effects
- Risk of severe circulatory adverse effects with exceeded dose
- Control elevated blood pressure with clonidine i.v.
- Treat bradycardia with atropine
