Indications
- Psoriasis (excluding widespread plaque psoriasis)
- Recalcitrant dermatoses
- Lichen planus
- Discoid lupus erythematosus
- Treatment for skin conditions not responding to milder steroids
Composition
- Clobetasol Propionate Cream: 0.05% w/w
- Clobetasol Propionate Ointment: 0.05% w/w
- Clobetasol Propionate Scalp Application: 0.05% w/w
- Clobetasol Propionate Shampoo: 0.05% w/w
- Clobetasol Propionate Lotion: 0.05% w/w
- Clobetasol Propionate Spray: 0.05% w/w
Pharmacology
Anti-inflammatory, antipruritic and vasoconstrictive properties with multiple mechanisms to inhibit late phase allergic reactions
Dosage
- Cream, Ointment:
- Apply twice daily for 2 consecutive weeks
- Limit maximum weekly dose to 50g/week
- For children, limit courses to five days and review weekly
- Spray: Apply once or twice daily to scalp, not exceeding 50 ml weekly
- Shampoo: Apply to dry scalp once a day, limit to 4 consecutive weeks, not recommended under 18 years
- Scalp Solution: Apply once or twice daily to affected areas of the scalp, not exceeding 50 ml weekly, use for brief periods only
Administration
Cutaneous, creams for moist or weeping surfaces, ointments for dry or scaly lesions
Interaction
Co-administered drugs that inhibit CYP3A4 can increase systemic exposure
Contraindications
- Rosacea, acne vulgaris, perioral dermatitis
- Hypersensitivity to the preparation
- Not indicated for primarily infected skin lesions caused by fungi or bacteria
- Not for dermatoses in children under one year of age
Side Effects
- Features of hypercorticism with prolonged use or treatment of extensive areas
- Local atrophic changes in the skin with prolonged and intensive treatment
- Rare instances of provoking pustular form of psoriasis
- Reports of pigmentation changes and hypertrichosis
Pregnancy & Lactation
Limited data in pregnant women, caution advised during lactation
Precautions & Warnings
- Avoid long-term continuous therapy, particularly in infants and children
- Atrophic changes on the face after prolonged treatment
- Care to prevent entry into the eyes when applied to eyelids
- Hazardous in psoriasis, careful patient supervision is important
- Use appropriate anti-microbial therapy when treating infected lesions
Use in Special Populations
Avoid long-term continuous therapy in infants and children under 12 years
Overdose Effects
Acute overdosage is unlikely, features of hypercortisolism may occur with chronic over-dosage or misuse
