Indications
- Treatment of mild, moderate or severe heart failure
- Conjunction with digitalis, diuretics, and ACE inhibitor
- Reduction of disease progression
- Use in patients unable to tolerate an ACE inhibitor
- Use in patients not receiving digitalis, hydralazine or nitrate therapy
Pharmacology
- Non-selective antagonism of β-adrenergic receptors
- α-adrenergic antagonistic activity
- Antiproliferative properties
- Scavenger of reactive free oxidant radicals
- Treatment of hypertension, angina pectoris, and congestive heart failure
Dosage & Administration
- Initial dosage in hypertension: 12.5 mg once daily
- Recommended dosage in angina pectoris: 12.5 mg twice daily for the first 2 days
- Recommended dosage in heart failure: 3.125 mg twice daily, increased at intervals to highest dose tolerated
Interaction
- Significantly increased levels of digoxin when taken together
- 60% decrease in Cmax and AUC with rifampin pretreatment
- No alteration of in vitro plasma protein binding of warfarin by vedilol
- β-receptor antagonists potentiate the pressor reaction which may follow the sudden withdrawal of treatment with clonidine
Contraindications
- Decompensated heart failure requiring intravenous inotropic therapy
- Bronchial asthma or related bronchospastic conditions
- Second or third-degree AV block
- Sick sinus syndrome without a permanent pacemaker
- Cardiogenic shock or severe bradycardia

Pregnancy & Lactation
- Not to be used during breast-feeding
- No established safety and efficacy in children
- Should not be used during pregnancy
- Carvedilol crosses the placental barrier
Precautions & Warnings
- Take caution in hepatic impairment
- Monitor clinical status for 2-3 hours after initiation and after increasing each dose
- Ensure that renal function and heart failure are not deteriorating before increasing the dose
