Description
- SGLT-2 inhibitor for improving glycemic control in adults with type 2 diabetes mellitus
- Reduces the risk of cardiovascular death in adult patients with type 2 diabetes mellitus and established cardiovascular disease
- Sodium-glucose Cotransporter-2 (SGLT2) Inhibitor
Dosage & Administration
- Recommended dose: 10 mg once daily in the morning, with or without food
- Dose may be increased to 25 mg once daily in patients tolerating the medication
- Correction of volume depletion prior to initiation recommended
Pharmacology
- Empagliflozin is a sodium glucose co-transporter-2 (SGLT-2) inhibitor
- Reduces renal absorption of glucose, leading to increased glucose excretion and reduced hyperglycemia
- Contributes to weight loss and reduces blood pressure
Indications
- Adjunct to diet and exercise for improving glycemic control in adults with type 2 diabetes mellitus
- Reduction of risk of cardiovascular death in adult patients with type 2 diabetes mellitus and established cardiovascular disease
Interaction
- Co-administration with diuretics results in increased urine volume
- Increased risk of hypoglycemia when co-administered with insulin or insulin secretagogues
- SGLT2 inhibitors lead to positive urine glucose tests and unreliable 1,5-AG assay in monitoring glycemic control
Contraindications
- History of serious hypersensitivity reaction to Empagliflozin or any of its ingredients
- Severe renal impairment
- End-stage renal disease or dialysis
Side Effects
- Common: Urinary tract infections and female genital mycotic infections
- Other common side effects: Dehydration, hypotension, weakness, dizziness, increased thirst
Pregnancy & Lactation
- Limited data on use in pregnant women, potential benefits should justify potential risk to the fetus
- Not recommended during breastfeeding
Precautions & Warnings
- Assessment of renal function recommended prior to initiation
- Not initiated in patients with eGFR less than 45 ml/min/1.73 m²
- No dose adjustment needed in patients with eGFR ≥ 45 ml/min/1.73 m²
Overdose Effects
- Supportive measures recommended in case of overdose
- Hemodialysis efficacy not studied
