Pharmacology
- Anti-inflammatory action
- Inhibition of Phospholipase A2 enzyme
- Decrease in the release of certain immune system chemicals
- Immunosuppressive action
Dosage & Administration
- Adult dosage up to 120 mg/day for acute disorders
- Maintenance doses in most conditions within the range of 3-18 mg/day
- Smallest effective dose should be used for rheumatoid arthritis
- High doses of 48-72 mg/day for bronchial asthma acute attack
- Children dosage usually lies in the range 0.25-1.5 mg/kg/day
- Alternate day administration may be appropriate
- Guidance for juvenile chronic arthritis, nephrotic syndrome, and bronchial asthma doses
Interaction
- Metabolized in the liver
- Recommendation to increase maintenance dose if liver enzyme inducers are co-administered
- Possibility to reduce maintenance dose for drugs which inhibit liver enzymes
Contraindications
- Hypersensitivity to any of the ingredients
- Patients receiving live virus immunization

- Crosses the placenta

- Unlikely systemic effects in the infant with doses up to 50 mg daily
Precautions & Warnings
- Special caution for cardiac disease or congestive heart failure
- Necessity for patient monitoring in various clinical conditions
- Potential salt and water retention
- Increased excretion of potassium
- Dietary salt restriction and potassium supplementation may be necessary
- Caution in gastritis, oesophagitis, diverticulitis, ulcerative colitis
- Special caution in diabetes mellitus, osteoporosis, myasthenia gravis, renal insufficiency
- Caution in emotional instability, psychotic tendency, epilepsy
- Previous corticosteroid-induced myopathy
- Attention in liver failure, hypothyroidism, and cirrhosis
- Special caution for ocular herpes simplex
Use in Special Populations
- Increased monitoring and dosage adjustment in hepatic impairment
- No special precautions for renally impaired patients
- No special precautions for the elderly
