Indications
- Prophylaxis of angina pectoris
- Not useful in aborting an acute anginal episode
Pharmacology
- Causes relaxation of vascular smooth muscle
- Produces vasodilator effect on peripheral arteries and veins
- Reduces left ventricular end-diastolic pressure
- Reduces systemic vascular resistance and arterial pressure
Dosage & Administration
- Dosage should be adjusted according to individual patient requirements and response
- One 2.6 mg sustained released tablet or capsule in morning and evening for adults
- Should be taken on an empty stomach
Interaction
- May increase antihypertensive properties of vasodilators, calcium antagonists, beta-adrenergic blockers
- Concomitant use with tricyclic antidepressants and alcohol may cause high blood pressure
- Concomitant use with phosphodiesterase type 5 (PDE5) inhibitors may cause fall in blood pressure
- Aspirin decreases the clearance and enhances the hemodynamic effects
- May reduce the pharmacologic effects of heparin
- Increases the bioavailability of dihydroergotamine
Contraindications
- Known hypersensitivity to nitroglycerin, other organic nitrates, or nitrites
- Acute myocardial infarction
- Marked anaemia
- Head trauma
- Cerebral haemorrhage
- Closed angle glaucoma
Side Effects
- Headache (at the onset of treatment)
- Tachycardia
- Postural hypotension and syncope
- Cyanosis
- Methaemoglobinaemia
Pregnancy & Lactation
- No evidence on safety in pregnancy and lactation
- Should be administered to pregnant women and nursing mothers only if considered essential by the physician
Precautions & Warnings
- Use with caution in patients predisposed to closed-angle glaucoma
- Abrupt discontinuation may lead to exacerbation of symptoms
- Dosage should be reduced gradually over several days when discontinuing long term treatment
- Particular attention required during early days of acute myocardial infarction
Overdose Effects
- Peripheral vasodilation with a fall in blood pressure
- Reflex tachycardia
- Monitoring of cardiac function and supportive measures required
- Circulating plasma volume should be increased by substitution of fluid
- Consideration of vasopressors in life-threatening situations
