Indications
- Treatment of peri-operative hypertension
- Controlling congestive heart failure in acute myocardial infarction
- Treatment of angina pectoris
- Induction of intraoperative hypotension
Pharmacology
- Relaxation of vascular smooth muscle
- Dilatation of peripheral arteries and veins
- Reduction of left ventricular end-diastolic pressure
- Reduction of pulmonary capillary wedge pressure
- Reduction of systemic vascular resistance
Dosage & Administration
- Not for direct intravenous injection
- Initial dilution of 50 mg nitroglycerin into 500 ml Dextrose (5%) Injection or Sodium Chloride Injection (0.9%)
- Maintenance dilution to limit fluids given to the patient
- Maximum concentration of the solution not to exceed 400 mcg/ml
Interaction
- Additive vasodilating effects with other vasodilators
- Pseudoagglutination and hemolysis when administered through the same infusion set as blood
- Interference with the anticoagulant effect of heparin
Contraindications
- Allergic reaction to organic nitrates
- Patients with pericardial tamponade, restrictive cardiomyopathy, or constrictive pericarditis
Side Effects
- Dose-related adverse reactions
- Headache, lightheadedness, hypotension
- Syncope, crescendo angina, rebound hypertension
- Rarely, methemoglobinemia
Pregnancy & Lactation
- Pregnancy category C
- Caution when administered to a nursing mother
Precautions & Warnings
- Severe hypotension and shock may occur
- Caution in volume-depleted or hypotensive patients
- Aggravation of angina caused by hypertrophic cardiomyopathy
- Development of tolerance
Use in Special Populations
- Safety and effectiveness in children have not been established
Overdose Effects
- Vasodilatation, venous pooling, reduced cardiac output, and hypotension
- Manifestations include increased intracranial pressure, confusion, fever, vertigo, palpitation, visual disturbances, nausea, and more
