Indications

  • Treatment of upper respiratory tract infections
  • Treatment of lower respiratory tract infections
  • Treatment of ear infections
  • Treatment of eye infections
  • Treatment of skin and soft tissue infections
  • Treatment of gastrointestinal tract infections
  • Prophylaxis for pre and post-operative, trauma, burns, rheumatic fever
  • Treatment of osteomyelitis, diptheria, scarlet fever, whooping cough

Description

  • Belongs to the macrolides group of antibiotics
  • Exerts bacteriostatic or bactericidal action based on specific interference with ribosomal protein synthesis
  • Used for treatment of infections caused by penicillinase producing staphylococci
  • Particularly susceptible to Gram (+)ve: Cocci and bacilli, Gram (-)ve: Neisseria catarhalis, N.meningitis, N.gonorrhoea, Haemophilus influenzae, Brodetella, H. pertussis, Mycoplasma pneumoniae, Legionella pneumophila, Bacteroides fragilis, B.oralis, B.melaninogenicus, Fusobacterium, Chlamydia trachomatis, Treponema pallidum

Pharmacology

  • Inhibits microsomal protein synthesis in susceptible organisms
  • Specific binding to the 50S subunit or 70S ribosome in susceptible organisms
  • Active against many Gram-positive bacteria, some Gram-negative bacteria, mycoplasmas, and chlamydia
  • Absorption rates diminished by the presence of food
  • Widely distributed throughout body tissue and fluids with some retention in the liver and spleen
  • Excreted in the urine and bile

Dosage & Administration

  • Adult and Children over 8 years: 250-500 mg every six hours for mild to moderate infections
  • Elderly: No special dosage recommendation
  • Children aged 2 to 8 years: 250 mg every six hours or 30-50 mg/kg body weight per day divided into four equal dosages
  • Infants and Children up to 2 years: 500 mg in divided doses or 30-50 mg/kg body weight in divided doses

Interaction

  • Potential interaction with theophylline resulting in increased serum theophylline levels and potential theophylline toxicity
  • Recommendation to reduce the dose of theophylline in such cases

Contraindications

  • Known hypersensitivity to Erythromycin

Side Effects

  • Rare allergic reactions, occasional abdominal discomfort, nausea, vomiting
  • Discomfort usually subsides after a few days without reducing the dosage

Pregnancy & Lactation

  • No evidence of human teratogenicity or toxicity in clinical and laboratory studies
  • Caution advised in pregnant patients and lactating mothers due to placental barrier crossing and excretion in breast milk

Precautions & Warnings

  • Care required in patients with impaired hepatic function as the product is excreted principally in the bile

Overdose Effects

  • Discontinue product in case of overdose
  • Prompt elimination of unabsorbed drug and institution of appropriate measures recommended
  • Not removed by peritoneal dialysis or haemodialysis

Reconstitution

  • Directions for reconstituting suspension
  • Shake the bottle to loosen powder
  • Add boiled and cooled water to the dry powder of the bottle
  • Shake well after each addition until all the powder is in suspension
  • Reconstituted suspension should be stored in a cool and dry place, preferably in the refrigerator and unused portion should be discarded after 7 days

  • Keep below 25°C temperature, away from light & moisture
  • Keep out of the reach of children
