Indications

  • Prophylaxis of angina pectoris

Pharmacology

  • Relaxation of vascular smooth muscle
  • Vasodilator effect on peripheral arteries and veins
  • Peripheral pooling of blood
  • Decreases venous return to the heart
  • Reduces left ventricular end-diastolic pressure
  • Reduces systemic vascular resistance

Dosage & Administration

  • Adjustable dosage according to patient response
  • One Nitroglycerin 2.6 mg sustained released tablet in morning and evening
  • Empty stomach intake

Interaction

  • May increase antihypertensive properties of vasodilators, calcium antagonists, beta-adrenergic blockers
  • Contraindicated with tricyclic antidepressants and alcohol
  • Contraindicated with phosphodiesterase type 5 (PDE5) inhibitors
  • Aspirin interaction
  • Heparin interaction
  • Increase bioavailability of dihydroergotamine

Contraindications

  • Known hypersensitivity to nitroglycerin or other organic nitrates
  • Acute myocardial infarction
  • Marked anaemia
  • Head trauma
  • Cerebral haemorrhage
  • Closed angle glaucoma

  • Tachycardia
  • Postural hypotension
  • Syncope
  • Cyanosis
  • Methaemoglobinaemia

Pregnancy & Lactation

  • No evidence for safety in pregnancy and lactation
  • Use with caution and physician's assessment

Precautions & Warnings

  • Caution in closed-angle glaucoma patients
  • Abrupt discontinuation leads to symptom exacerbation
  • Gradual dosage reduction when discontinuing long term treatment
  • Particular attention during early days of acute myocardial infarction

  • Peripheral vasodilation
  • Fall in blood pressure
  • Reflex tachycardia
  • Monitoring of cardiac function
  • General supportive measures
  • Administration of vasopressors in life-threatening situations
