Indications

  • Indicated for the treatment of hypertension
  • Lowers blood pressure
  • Reduces risk of cardiovascular events

Pharmacology

  • Telmisartan blocks vasoconstrictor and aldosterone-secreting effects of angiotensin II
  • Hydrochlorothiazide is a thiazide diuretic affecting renal tubular mechanisms

Dosage & Administration

  • Initiate with Telmisartan monotherapy 80 mg once daily
  • Dose can be titrated up to 160 mg/25 mg after 2 to 4 weeks
  • May be administered with other antihypertensive drugs

Interaction

  • Caution advised when taking with aliskiren, digoxin, lithium, other high blood pressure medicines
  • May interfere with certain laboratory tests

Contraindications

  • Contraindicated in patients with known hypersensitivity to Telmisartan or Hydrochlorothiazide
  • Do not co-administer with Aliskiren in patients with Diabetes

Side Effects

  • Common side effects include dizziness, drowsiness, tired feeling, back pain, nausea, diarrhea, stomach pain
  • Other adverse events include allergy, fever, leg pain, chest pain, insomnia, elevations of liver enzymes

Pregnancy & Lactation

  • Pregnancy Category D
  • Discontinue as soon as pregnancy is detected
  • Potential adverse effects on nursing infants

Precautions & Warnings

  • Caution advised for patients with kidney disease, liver disease, dehydration, gout, lupus
  • May affect blood sugar levels
  • May cause dizziness, dehydration

Use in Special Populations

  • Safety and effectiveness not established in pediatric patients
  • Cautious dose selection for elderly patients
  • Close medical supervision for patients with biliary obstructive disorders or hepatic insufficiency

Overdose Effects

  • Manifestations may include hypertension, dizziness, tachycardia, hypokalemia, dehydration
  • Telmisartan not removed by hemodialysis
