Indications
- Treatment of essential hypertension in adults
- Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease or type 2 diabetes mellitus with documented target organ damage
Pharmacology
- Telmisartan is a non-peptide angiotensin-ll receptor antagonist
- Blocks vasoconstrictor and aldosterone secreting effects of angiotensin-ll
- Has much greater affinity for the AT1 receptor than for the AT2 receptor
Dosage & Administration
- Usual starting dose of 40 mg once a day
- Blood pressure response is dose-related over the range of 20 to 80 mg
- Antihypertensive effect is apparent within 2 weeks
- Maximal reduction is generally attained after 4 weeks
- No initial dosage adjustment is necessary for elderly patients or patients with renal impairment
- Pediatric use not established
- Not recommended in patients ≥75 years old or with hepatic impairment
- Monitor carefully in patients with biliary obstructive disorders or hepatic insufficiency
Interaction
- Increased risk of renal impairment and loss of antihypertensive effect when co-administered with NSAIDS
- Avoid co-administration with aliskiren in patients with diabetes
Contraindications
- Known hypersensitivity to this product or any of its components
Side Effects
- Common side effects in hypertensive patients include sinus pain, congestion, back pain, diarrhea
- Common side effects in CV risk reduction patients include intermittent claudication and skin ulcer
Pregnancy & Lactation
- Pregnancy Category C (first trimester) and D (second and third trimester)
- Potential adverse effects on nursing infant
Precautions & Warnings
- Avoid fetal or neonatal exposure
- Hypotension
- Monitor carefully in patients with impaired hepatic or renal function
- Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker
Overdose Effects
- Manifestations of overdosage could include hypotension, dizziness, tachycardia, bradycardia, increase in serum creatinine, acute renal failure
