Indications

  • Treatment of essential hypertension in adults
  • Reduction of cardiovascular morbidity in adults with atherothrombotic cardiovascular disease or type 2 diabetes mellitus with documented organ damage

Pharmacology

  • Non-peptide angiotensin-II receptor antagonist
  • Blocks vasoconstrictor and aldosterone secreting effects of angiotensin-II
  • Independent of pathways for angiotensin-II synthesis
  • Does not inhibit ACE
  • Does not affect the response to bradykinin
  • Does not bind to or block other hormone receptors or ion channels known to be important in cardiovascular regulation

Dosage & Administration

  • Usual starting dose of 40 mg once a day
  • Blood pressure response is dose-related (20 to 80 mg)
  • Most antihypertensive effect apparent within 2 weeks
  • May be administered with other antihypertensive agents
  • May be administered with or without food
  • Not recommended for patients ≥75 years old or with hepatic impairment
  • Pediatric use safety and effectiveness not established
  • No initial dosage adjustment necessary for elderly patients or patients with renal impairment
  • Monitor carefully and up titrate slowly in patients with biliary obstructive disorders or hepatic insufficiency

Interaction

  • Increased risk of renal impairment and loss of antihypertensive effect with NSAIDs
  • Co-administration with aliskiren should be avoided in patients with diabetes

Contraindications

  • Known hypersensitivity to the product or any of its components

Side Effects

  • Common side effects include sinus pain and congestion, back pain, diarrhea (hypertensive patients)
  • Common side effects include intermittent claudication and skin ulcer (patients of cardiovascular risk reduction)

Pregnancy & Lactation

  • Pregnancy Category C (first trimester) and D (second and third trimester)
  • Potential adverse effects on the nursing infant
  • Consider discontinuing nursing or drug, taking into account the importance of the drug to the mother

Precautions & Warnings

  • Avoid fetal or neonatal exposure
  • Hypotension
  • Monitor carefully in patients with impaired hepatic or renal function
  • Avoid concomitant use of an ACE inhibitor and angiotensin receptor blocker

Overdose Effects

  • Likely manifestations include hypotension, dizziness, tachycardia, bradycardia, increase in serum creatinine, and acute renal failure
