Description

Indication
Adjuvant treatment of postmenopausal women with hormone receptor-positive early breast cancer, first-line treatment of postmenopausal women with hormone receptor-positive or hormone receptor unknown locally advanced or metastatic breast cancer, second-line treatment of advanced breast cancer in postmenopausal women with disease progression following tamoxifen therapy.
Pharmacology
Non-steroidal aromatase inhibitor that suppresses estrogen biosynthesis in peripheral tissues and in the cancer tissue itself by specifically inhibiting the aromatase enzyme.
Dosage
One tablet 1 mg once a day for advanced breast cancer; duration for adjuvant treatment in early breast cancer is unknown.
HepaticImpairment
No dosage changes for mild-to-moderate impairment; not studied in severe impairment.
RenalImpairment
No dosage changes necessary.

Safety and efficacy not established.
GeriatricUse
Pharmacokinetics not affected by age.
Interaction
- Reduces plasma concentration if co-administered with tamoxifen; should not be used with estrogen-containing therapies; unlikely to affect other drugs via cytochrome P450 inhibition.

- Common adverse reactions: hot flashes, asthenia, arthritis, pain, hypertension, depression, nausea, vomiting, rash, osteoporosis, fractures, back pain, insomnia, headache, bone pain, peripheral edema, increased cough, dyspnea, pharyngitis, lymphedema.

PrecautionsWarnings
- Increased incidence of ischemic cardiovascular events in women with pre-existing ischemic heart disease
- Mean decrease in bone mineral density compared to baseline
- More patients reported elevated serum cholesterol
OverdoseEffects
Well tolerated up to 60 mg in healthy male volunteers and up to 10 mg daily in postmenopausal women with advanced breast cancer; no specific antidote, treatment must be symptomatic.
