Indications
- Reduce total cholesterol and LDL cholesterol in patients with heterozygous and homozygous familial hypercholesterolaemia
- Reduce elevated cholesterol and triglycerides in patients with mixed dyslipidemia (Fredrickson Type Ia and Ib)
- Treatment of patients with elevated serum triglyceride levels in hypertriglyceridaemia (Fredrickson Type IV)
- Treatment of patients with dysbetalipoproteinaemia (Fredrickson Type III)
- Reduce cardiac ischaemic events in patients with asymptomatic or mild to moderate symptomatic coronary artery disease with elevated LDL-cholesterol level
- Reduce total and LDL-cholesterol concentrations in patients with hypercholesterolemia associated with or exacerbated by diabetes mellitus or renal transplantation
Pharmacology
- Mode of action: Selective inhibitor of HMG-CoA reductase
- Absorption: Rapidly absorbed after oral administration; maximum plasma concentrations occur within 1 to 2 hours
- Distribution: Mean volume of distribution is approximately 381 liters; 98% bound to plasma proteins
- Metabolism: Extensively metabolized to ortho- and parahydroxylated derivatives and various beta-oxidation products
- Excretion: Eliminated primarily in bile following hepatic and/or extra-hepatic metabolism
Dosage and administration
- Adults:
- Primary hypercholesterolaemia and combined hyperlipidaemia: Usually 10 mg once daily; may be increased at intervals of at least 4 weeks to max. 80 mg once daily
- Familial hypercholesterolaemia: Initially 10 mg daily, increased at intervals of at least 4 weeks to 40 mg once daily; if necessary, further increased to max. 80 mg once daily
- Prevention of cardiovascular events: Initially 10 mg once daily adjusted according to response
- Children:
- Primary hypercholesterolaemia and combined hyperlipidaemia: Initially 10 mg once daily, increased if necessary at intervals of at least 4 weeks to max. 20 mg once daily
- Familial hypercholesterolaemia: Initially 10 mg once daily, increased if necessary at intervals of at least 4 weeks to usual max. 80 mg once daily

Contraindications
- Hypersensitivity to any component of this medication
- Active liver disease or unexplained persistent elevations of serum transaminases
- History of serious adverse reaction to prior administration of HMG-CoA reductase inhibitors

Pregnancy and lactation
- Pregnancy: Contraindicated; safety in pregnant women not established; rare reports of congenital anomalies; animal studies show toxicity to reproduction
- Lactation: Not known whether excreted in human milk; contraindicated during breastfeeding
Precautions and warnings
- Liver function tests should be performed before the initiation of treatment and periodically thereafter
- Use with caution in patients who consume substantial quantities of alcohol or have a history of liver disease
- Discontinue therapy if markedly elevated CPK levels occur or myopathy is diagnosed or suspected
Use in special populations
- Geriatric: Plasma concentrations are higher in healthy elderly subjects than in young adults
- Pediatric: Pharmacokinetic data not available
- Gender: No clinically significant difference in LDL-C reduction between men and women
- Renal insufficiency: No influence on the plasma concentrations or LDL-C reduction
- Hemodialysis: Not expected to significantly enhance clearance
- Hepatic insufficiency: Plasma concentrations are markedly increased in patients with chronic alcoholic liver disease
Overdose effects
Specific treatment not available; treat symptomatically; monitor liver function tests and serum CK levels
