Indications

  • Prophylaxis of angina pectoris

Pharmacology

  • Relaxation of vascular smooth muscle
  • Vasodilator effect on peripheral arteries and veins

Dosage & Administration

  • Adjustable dosage according to patient's requirement
  • Morning and evening dosage
  • Empty stomach intake

Interaction

  • Increase of antihypertensive properties of vasodilators, calcium antagonists, beta-adrenergic blockers
  • Concomitant use with tricyclic antidepressants and alcohol may cause high blood pressure
  • Fall in blood pressure when used with phosphodiesterase type 5 (PDE5) inhibitors
  • Decrease in clearance and enhancement of hemodynamic effects with aspirin
  • Reduction of pharmacologic effects of heparin when used concomitantly
  • Increase in bioavailability of dihydroergotamine with nitrates

Contraindications

  • Hypersensitivity to nitroglycerin or other organic nitrates
  • Acute myocardial infarction
  • Marked anaemia
  • Head trauma
  • Cerebral haemorrhage
  • Closed angle glaucoma

Side Effects

  • Headache
  • Tachycardia
  • Postural hypotension and syncope
  • Cyanosis and methaemoglobinaemia

Pregnancy & Lactation

  • No evidence for safety in pregnancy and lactation
  • Use with caution and only if considered essential by physician

Precautions & Warnings

  • Use with caution in patients predisposed to closed-angle glaucoma
  • Abrupt discontinuation may lead to symptom exacerbation
  • Gradual dosage reduction upon discontinuation of long term treatment
  • Hemodynamic monitoring during early days of acute myocardial infarction treatment

Overdose Effects

  • Peripheral vasodilation with a fall in blood pressure and reflex tachycardia
  • Monitoring of cardiac function and general supportive measures
  • Circulating plasma volume increase by fluid substitution
  • Consideration of vasopressor administration in life-threatening situations
