Indications

  • Prophylaxis of angina pectoris
  • Not useful for aborting acute anginal episode

Pharmacology

  • Causes relaxation of vascular smooth muscle
  • Produces vasodilator effect on peripheral arteries and veins
  • Reduces left ventricular end-diastolic pressure
  • Reduces systemic vascular resistance

Dosage & Administration

  • Dosage adjusted according to patient's requirement and response
  • One 2.6mg sustained release tablet in morning and evening
  • Empty stomach intake

Interaction

  • May increase antihypertensive properties of vasodilators
  • May cause high blood pressure with tricyclic antidepressants and alcohol
  • May cause fall in blood pressure with PDE5 inhibitors like sildenafil
  • Aspirin decreases clearance and enhances the effects
  • May reduce pharmacologic effects of heparin
  • Increases bioavailability of dihydroergotamine

Contraindications

  • Known hypersensitivity to nitroglycerin
  • Contraindicated in acute myocardial infarction, marked anaemia, head trauma, cerebral haemorrhage, closed angle glaucoma

Side Effects

  • Headache at onset, tachycardia, postural hypotension, syncope, cyanosis, methaemoglobinaemia

Pregnancy & Lactation

  • No evidence regarding safety in pregnancy and lactation
  • Use in pregnant and nursing mothers only if essential

Precautions & Warnings

  • Use with caution in patients predisposed to closed-angle glaucoma
  • Abrupt discontinuation may exacerbate symptoms
  • Gradual dosage reduction when discontinuing long term treatment
  • Special attention needed during early days of acute myocardial infarction

Overdose Effects

  • Peripheral vasodilation with fall in blood pressure
  • Reflex tachycardia
  • Monitoring of cardiac function and supportive measures needed
  • Consideration of vasopressors in life-threatening situations
