Dosage

  • Available in 375 mg naproxen/20 mg esomeprazole
  • Available in 500 mg naproxen/20 mg esomeprazole
  • Dosage for Rheumatoid Arthritis, Osteoarthritis and Ankylosing Spondylitis
  • Dosage for Juvenile Idiopathic Arthritis in Adolescent Patients

Administration

  • Take at least 30 minutes before meals

Pharmacology

  • Consists of immediate release Esomeprazole Magnesium layer & enteric-coated Naproxen core
  • Mechanism of action of Naproxen is to inhibit prostaglandin synthesis
  • Esomeprazole suppresses gastric acid secretion by inhibiting the H+/k+ -ATPase

Interactions

  • Reduces antihypertensive effect of ACE inhibitors, diuretics & beta-blockers
  • May interfere with the absorption of drugs where gastric pH is important (e.g. Ketoconazole, iron salts, digoxin)

Contraindications

  • Known hypersensitivity to any component of the tablet or substituted benzimidazoles
  • History of asthmay urticaria or other allergic-type reactions after taking aspirin or other NSAIDs
  • Use during the peri-operative period in the setting of coronary artery bypass graft (CABG) surgery

Side Effects

  • Decreases occurrence of gastric ulcers and NSAID associated upper gastrointestinal adverse events compared to Naproxen alone
  • May be associated with a small increased risk of arterial thrombotic events
  • Reported adverse events include gastrointestinal issues and cardiovascular effects

Pregnancy & Lactation

  • Should not be given in first and second trimester of pregnancy unless benefits outweigh risks
  • Contradicted during the third trimester of pregnancy
  • Should not be used during breastfeeding

Precautions & Warnings

  • Should be avoided in combination with NSAIDs including cyclooxygenase-2 selective inhibitors
  • Use lowest effective dose for the shortest duration necessary
  • Patients with high age, concomitant use of anticoagulants, corticosteroids and other NSAIDs have increased risk of gastrointestinal complications

Special Populations

  • Elderly patients may require dosage adjustment
  • Not recommended for use in patients with moderate to severe or severe renal impairment
  • Monitor patients with mild to moderate hepatic impairment closely

Overdose Effects

  • Symptoms may include lethargy, dizziness, gastrointestinal issues, hypertension, and respiratory depression
  • No specific antidote known, treatment should be symptomatic and supportive
