Indications
- Adjunct to diet and exercise to lower blood glucose in patients with noninsulin dependent (Type II) diabetes mellitus (NIDDM)
- May be used concomitantly with metformin for inadequate glycaemic control
Pharmacology
- Decreases blood glucose concentration
- Stimulates release of insulin from pancreatic beta cells
- Improves sensitivity of beta cells to glucose stimulus
- Reduces basal hepatic glucose production
- Increases peripheral tissue sensitivity to insulin
- Glucose uptake stimulation
Dosage
- Governed by the desired blood sugar level
- Initial dose is 1 mg once daily
- Usual dose range in patients with well controlled diabetes is 1 to 4 mg daily
- Timing and distribution of doses are decided by the physician
- Doses titration based on regular blood sugar monitoring
- Secondary dosage adjustment for improved diabetes control
Administration
- Tablet must be swallowed with sufficient amount of liquid
Interaction
- Potentiates: ACE inhibitors, chloramphenicol, fluconazole, sulphonamide antibiotics
- Attenuates: Calcium channel blockers, corticosteroids, diuretics, phenothiazines
Contraindications
- Not suitable for insulin dependent (type I) diabetes mellitus
- Not suitable for diabetic ketoacidosis or diabetic coma
- Not for patients hypersensitive to glimepiride, other sulfonylureas, other sulfonamides
- Not for patients with severe hepatic dysfunction, severe impairment of renal function, or dialysis patients
Side Effects
- Hypoglycaemia
- Temporary visual impairment
- Nausea
- Vomiting
- Diarrhoea
- Abdominal pain
- Urticaria
- Fall in blood pressure
Pregnancy & Lactation
- Not to be taken during pregnancy; changeover to insulin is necessary
- Not to be taken by breastfeeding women
Precautions & Warnings
- Initial weeks of treatment may increase risk of hypoglycaemia
- Careful monitoring required
Overdose Effects
- Produces hypoglycaemia
- Mild symptoms without loss of consciousness treated with oral glucose
- Severe reactions with coma or seizure require immediate hospitalization
