Therapeutic Class

  • Sulfonylureas

Indications

  • Adjunct to diet and exercise to lower blood glucose in patients with noninsulin dependent (Type II) diabetes mellitus (NIDDM)
  • May be used concomitantly with metformin for inadequate glycaemic control

Pharmacology

  • Decreases blood glucose concentration
  • Stimulates release of insulin from pancreatic beta cells
  • Improves sensitivity of beta cells to glucose stimulus
  • Reduces basal hepatic glucose production
  • Increases peripheral tissue sensitivity to insulin
  • Glucose uptake stimulation

Dosage

  • Governed by the desired blood sugar level
  • Initial dose is 1 mg once daily
  • Usual dose range in patients with well controlled diabetes is 1 to 4 mg daily
  • Timing and distribution of doses are decided by the physician
  • Doses titration based on regular blood sugar monitoring
  • Secondary dosage adjustment for improved diabetes control

Administration

  • Tablet must be swallowed with sufficient amount of liquid

Interaction

  • Potentiates: ACE inhibitors, chloramphenicol, fluconazole, sulphonamide antibiotics
  • Attenuates: Calcium channel blockers, corticosteroids, diuretics, phenothiazines

Contraindications

  • Not suitable for insulin dependent (type I) diabetes mellitus
  • Not suitable for diabetic ketoacidosis or diabetic coma
  • Not for patients hypersensitive to glimepiride, other sulfonylureas, other sulfonamides
  • Not for patients with severe hepatic dysfunction, severe impairment of renal function, or dialysis patients

Side Effects

  • Hypoglycaemia
  • Temporary visual impairment
  • Nausea
  • Vomiting
  • Diarrhoea
  • Abdominal pain
  • Urticaria
  • Fall in blood pressure

Pregnancy & Lactation

  • Not to be taken during pregnancy; changeover to insulin is necessary
  • Not to be taken by breastfeeding women

Precautions & Warnings

  • Initial weeks of treatment may increase risk of hypoglycaemia
  • Careful monitoring required

Overdose Effects

  • Produces hypoglycaemia
  • Mild symptoms without loss of consciousness treated with oral glucose
  • Severe reactions with coma or seizure require immediate hospitalization
